Plasma ergonovine was determined by high perfor-mance liquid chromatography and fluorescence detec-tion. Absolute recoveries of ergonovine was 70%. How-ever coefficients of variation of relative peak height of ergonovine versus internal standard was 1.0%. The detection limit for ergonovine was 5ng/ml. Thus our method was very stable and sensitive. Concentration of ergonovine was measured in 14 patients with occulusive coronary spasm (Group A) or nineteen patients with non-occlusive, diffuse spasm (Group B) after ergonovine administration. When 0. 1mg, 0.2mg, 0.4mg of ergono-vine was intravenonsly administered to the group B, plasma ergonovine levels were 7.9±3.2, 11.3±3.3, 17.3±6.0 (Mean±SD ng/ml), respectively. The levels of ergonovine producing occlusive coronary spasm was less than 1Ong/ml in 12 cases of the group A, while spasm was induced at more than 2Ong/ml in 2 cases of the group A. Our study showed that provocative test by administration of ergonovine (0.4mg) might under-diagnose variant form of angina in some cases, because this method delivered ergonovine to coronary vascular bed at insufficient and low ergonovine concentration in such cases.
