Twenty-eight patients with chronic ventricular arrhythmias were treated with oral mexiletine 300~ 500mg per day. The antiarrhythmic effects of mexiletine were evaluated by 24 hours Holter ECG and/or exercise test. Suppression of ventricular ectopic beats (greater than 50%) was noted in 7 of 14 patients (50%) in Holter ECG study, and 10 of 16 patients (63%) in exercise study. In 11 patients, the comparison of effects of oral disopy-ramide 300mg per day and mexiletine 450mg per day was also studied. At these dosages mexiletine is more effective than disopyramide, and disopyra-mide-resistant ventricular ectopic beats were sup-pressed by mexiletine in 2 patients. No complica-tions occured in cardiovascular system, however, adverse effects occurred in 8 patients (29%); these were gastrointestinal, neurologic and dermatologic (eruption).

Mexiletine is useful and effective in patients having ventricular arrhythmias especially provo-cated by exercise and resistant to other antiarrhy-thmic agents.
