This report describes a case of a one-year-old girl of Arnold-Chiari type II malformation observed frequent central sleep apneas.

By means of polygraphic recording of EEG, ECG, FECO2 and tidal volume, the pathophysiology of the sleep apnea was examined. The sleep apnea much disturbed the periodical regular breathing in REM sleep rather than in non-REM sleep. The CO2 response during REM sleep was not observed while the ventilation reacted the CO2 level during non-REM sleep.

In order to decrease the sleep apnea, transcutane-ous electrical stimulaton (TES) was applied to the region 2-3cm bilateral from processus spinalis of the third thoracic vertebra. At one month after the start of the stimulation, the extent of sleep apnea duration was decreased significantly, furthermore, the CO2 response during non-REM sleep became much sensitive. The stimulation just at a time of sleep apnea attack recovered the periodical normal breathing.

It is suggested that the respiratory disorder of Arnold-Chiari type II malformation is due to the dysfunction of respiratory center in the meddula associated with the dysregulation of chemosensitivity and further that TES is useful to prevent the attack of sleep apnea.
