　うっ血性心不全の治療はジギタリスや利尿剤がその主体であったが，近年重症ならびに難治性心不全の治療対策の1つとして血管拡張剤による心室負荷軽減療法が注目されてきている。血管拡張剤には主として動脈系に作用し末梢抵抗血管を拡張させるPhentolamine,Phenox—benzamine，静脈系血管床に作用し容量血管を拡張させる亜硝酸剤，その両作用を有するSodium Nitropru—side,Prazocin等があり，心不全に対する有効性が指摘されている^1〜13）。

　1964年Bergstromら^14）がProstaglandin E₁（PGE₁）をヒトに点滴静注することにより，収縮期および拡張期血圧を低下させたとして以来種々の報告がある^14〜17）。PGE₁はProstaglandin Eシリーズの中でも最も強い血管拡張作用を有し，その作用は血管平滑筋に対して直接に作用し，カテコールアミン，アンジナテンシン，バゾプレッシンの作用を抑制させるものと考えられている^15,16）。血管拡張作用以外に腎では水利用，ナトリウム利尿作用，血小板凝集抑制作用，抗不整脈作用が報告されている^16〜20）。これらの作用に注目し，今回我々は14例の重症心不全患者に対するPGE₁の急性効果を血行力学的に検討し，その有効性を示唆する知見をえたので文献的考察を加えて報告する。

The effectes of prostaglandin E1(PGE₁) on hemo-dynamics, blood gas and metabolism were studiedin 14 patients with severe congestive heart failure, PGE₁ was administered intravenously 0.1μg/kg/minfor 60 minutes, Significant hemodynamic effects were obtained within 5 minutes, and persistcd for60 minutes after adnimistration. At peak effect, cardiac index increased from 2.03 L/min//ml to 3.50 L/min/ml (p<0.001), together with significant increase in stroke index and LV work index, mean blood pressure decreased from 109 mmHg to 84 mmHg (p<0.01), systemic vascular resistance index (4,024 to 1,896 dynes･sec･cm^-5･M²), mean pulmo-nary capillary wedge pressure (26 to 12 mmHg), mean pulmonary pressure (39 to 23 mmHg), mean right atrial pressure (7 to 4 mmHg), pulmo-nary vascular resistance index 529 to 266 dyes･sec･cm^-5･M²) and double product (14,222 to 10,415) decreased significantly, but heart rate remained unchanged. Infusion of PGE₁ for 60 minutes resulted in a significant decreae in arteriovenous oxygen difference (6.3 to 4.3 ml/dl), decrease but not significant in arterial oxygen pressure (87 to 77 mmHg), a significant increase in serum angiotensin I, aldosteron and adrenalin level, but no significant change of triglyceride and free fatty acid. PGE1 seemed to be a potent vasodilator with both acting as afterload and preload reduction and to be useful for the treatment of severe heart failure.
