虚血心におけるニトログリセリンの心筋局所灌流にたいする影響—RI心血管造影，心筋シンチグラムによる検討

Radionuclide Assessment of Nitroglycerin Influence on Regional Myocardial Perfusion in Ischemic Heart

　ニトログリセリンは，狭心症治療の最も有効な薬剤の1つとして，100年以上わたり使用されてきたが，その作用機序は，いまだ解明されていない点が多い。虚血心に対するニトログリセリンの作用は，その末梢血管拡張作用による心仕事量軽減の効果，および，冠血管拡張作用による冠血流の変化の2点より検討されてきた^1,2）。しかし，冠血流におよぼすニトログリセリンの影響に関しては，動物実験を除いて，全心筋の平均血流の変化のみが測定され^3,4），局所心筋の灌流状態の変動に関する報告は少ない。近年，核医学のめざましい進歩により，²⁰¹T1をはじめとするカリウム同族体の心筋シンチグラム製剤が開発され，非観血的に心筋局所灌流状態を観察することが可能となり^6），すでに虚血性心疾患を対象に，その有用性が多く報告されている^7,8）。

　われわれは，左室収縮異常（asynergy）を有する虚血性心疾患を対象に，RI心血管造影，および，²⁰¹Tl心筋シンチグラムをニトログリセリン舌下投与前後で行ない，ニトログリセリンの心機能，心筋局所灌流に対する影響を検討したので報告する。

To evaluate the effect of nitroglycerin (TNG) on ischemic heart, we employed ECG-synchronous cardiac pool imaging with Tc-99m labeled human serum albumin and myocardial perfusion scin-tigram with T1-201 in ten patients with coronary artery disease before and after administration of 0.6 mg of TNG. Left ventricular end-diastolic volume (EDV), end-systolic volume (ESV), ejec-tion fraction (EF) and left ventricular wall motion were obtained from cardiac pool imagings. Back-ground subtracted myocardial images and T1-uptake indexes in various myocardial segments were employed to evaluate regional myocardial perfusion.

In five patients in whom asynergy improved with TNG (improved group) compared to five who were not improved (unimproved group), the percent increase of EF was significant (13.8±3.1 vs 2.0±1.1%; p<0.01) without difference in per-cent changes in EDV (-18.1±2.9 vs -17.6±4.4 %), systolic blood pressure (-15.4±2.3 vs -23.6 ±8.6%) and double product (-10.6±8.6 vs -12.0 ±9.6%).
