左心室へ開口する冠動脈瘻の臨床

Clinical Features of Congenital Fistula of Coronary Artery to Left Ventricle

　大動脈弁または肺動脈弁閉鎖を伴わない先天性冠動脈瘻は比較的稀な疾患であるが，逆行性大動脈造影または選択的冠動脈造影により正確な診断が容易に下しうる現在になって，その報告は200例を越えるに至った^1）2）。ところで冠動脈瘻は右心系に開口することが圧倒的に多く^1）−6），左心系ことに左心室へ開口する冠動脈瘻の報告例はきわめて少ない^{1）7）−18）}。したがって冠動脈左室瘻の臨床像は未だ十分明らかにされていない。

　本論文の目的は右冠動脈が左心室に開口する冠動脈左室瘻の1自験例をもとに，これまでに報告された14症例の冠動脈左室瘻の臨床像を文献的に考案し，その臨床的特徴を明らかにすることである。

Congenital fistula of the coronary artery communicating with left ventricle in no asso-ciation with aortic atresia is a rare congenital heart disease and its clinical features have not yet been fully described. Therefore, one case, observed at Nagoya National Hospital, and four-teen other cases so far reported in the literature were compared and reviewed in order to present clinical manifestations of the entity of this disease.

Although ages ranged from 10 months to 62 years, the majority were under 20 years of age.
