H-V時間の変動を示した房室ブロックの1例

Fluctuating H-V Interval in a Patient with Transient Complete Heart Blook

　心電図上，P-Q時間の変動を見る場合，その多くはA-H間にあり，H-V時間は一定であるのが普通である^1）。しかしながら，ごく一部の例においては，H-V間のWenckebach周期^2）3）や間歇的左脚ブロック^4）5）時にH-V時間が変動することが知られている。

　今回，我々は一過性に完全房室ブロックを示した例において，洞調律時にH-V時間が突然延長する例を経験したので報告するとともに，そのメカニズムにつき若干の考察を加えてみた。

A patient with complete right bundle branch block with left axis deviation and a history of advanced A-V block manifesting two H-V inter-val was described.

In His bundle recordings, H-V interval was abruptly prolonged from 70 msec. to 100 msec. The prolongation of H-V interval was observed when the preceding H-H interval was less 900 msec. and the pre-preceding H-H interval was longer than the preceding H-H interval.
