手術後A-V Blockの回復過程にみられたSupernormal Conduction

Supernormal Atrioventricular Conduction Observed in the Process of Recovery from Surgical A-V Block

　心室中隔欠損の閉鎖や心内膜床欠損症の手術等に際し，刺激伝導系の損傷のためにA-V Blockが発生することがある。このような手術後のA-V Blockには永久的に残るものもあるが，しばしば一過性であり数日後には回復する場合もある。この回復過程については，ある時突然洞調律にもどることが多いが頻回に心電図をとってくわしく観察すると興味ある所見が得られることがある。最近われわれはFallot四徴症の根治手術後A-Vblockとなり，途中Supernormal ConductionによるVentricular Captureを伴なうIncomplete A-V disso—ciationをへて右脚Block，左軸偏位を呈する洞調律に回復した症例を経験したので報告する。

The conduction system is liable to injury by surgical procedures for correcting cardiac malforma-tions related with ventricular septum. Depressed A-V conduction induced by local ischemia or hy-poxia sometimes regain its normal conductivity. A case was presented in which supernormal atrioven-tricular conduction was observed in the process of recovery from A-V block induced by radical oper-ation of tetralogy of Fallot. The supernomal con-duction does not occur in the normal heart but only when conductivity is depressed.
