圧脈波用心拍出量計の臨床応用に関する研究—第1報，その理論と実験的検討

Experimental Evaluation of Beat-by-beat Estimates of Stroke Volume from the Arterial Presssure Wave Contour in Comparison with the Electromagnetic Flowmeter

　臨床上，心拍出量の測定は循環動態を知る重要な指標である。とくに心臓外科の術後管理，中でもその重症例の術後管理においては心拍出量の持続的測定は血圧測定と並んで，循環動態を知る重要な指標で治療上その意義は極めて大きいと考えられる。しかし心拍出量の持続的測定は積極的におこなわれていないのが現状である。一方その術後管理において動脈血圧を観血的に測定することは普及化の傾向にある。そこで著者らは観血的に測定する動脈血圧の脈波を利用して心拍出量を算出する場合の理論と，圧脈波用心拍出量計を試作し種々の循環動態下で実験的検討を加え，この方法が心臓外科の術後管理に応用できるかどうかを考察したので報告する。

We experimentally evaluated the method de-veloped by modified HERD's technique, cardiac output is related to a measure derived from the arterial pressure wave and the heart rate.

Beat-by-beat comparison of stroke volume estimates against estimates based on integration of the output of an electromagnetic flowmeter around the aorta shows a correlation coefficient of 0.829-0.949 under conditions of control and infusion of vesprin, vesprin + lowmolecular weight dextran, vesprin+lowmolcular weight dextran+isoproterenol and noradrenalin.
