人工弁の形態と弁抵抗—水力学的比較検討

A Hydraulic Study on Prosthetic Heart Valves

緒言

　いわゆるteaflet型人工弁は形態的に自然弁に近くまた機能的にも優れているとされていたが，1963年にいたり，その耐久性の限界が次第に認識されるようになった^1）2）。それ以後はStarr-Edwards^3），　Harken^4），Smeloff-Cutter^5），　Kay-Suzuki^6）など，自然弁の形態をまったく無視し機能面のみを考慮にいれた固定弁座をもつ人工弁が，数多く臨床に使用されている。人工弁はその種類によっておのおの長所と短所をあわせもっており，実際に数多くの人工弁からその一つを選択するとき各弁の特性を知ることは，心臓外科医にとって興味のある点であると同時に，人工弁移植をうける患者の手術予後を決定する大きな要素の一つとなる。

　今回の報告は，人工弁のもつ数多くの特性のうちから水力学的な面を選び，その一端としての弁抵抗を各種弁模型を用いて実験的に測定し比較検討したものである。

Prosthetic heart valve is now in common use, and great many cardiac patients have been enjoying their lives with full antivities. Considering the variety in type of prosthetic heart valves, it is apparant that each pros-thesis has its own advantages together with disadvantages in shape, mechanism, durabi-lity, and so on. It, therefore, is thought to be of a great importance for cardiovascular surgeons to understand the property of each prosthesis in detail. This study was proje-cted to clarify the characteristics of many prosthetic valves from the hydraulic stand point.
