Mean Spatial Vector Electrocardiographyに関する研究—第1編 邦人健常者のMean Spatial QRS, T Vector及びMean Spatial QRS-T Angleを中心として

Studies on the Mean Spatial Vector Electrocardiography (Ⅰ) With Special Reference to the Mean Spatial QRS, T Vectors and Mean Spatial QRS-T Angle in Normal Subjects

Ⅰ．緒言

　臨床心電に於ては不整脈を除けばQRS波とT波の変化の研究が最も重要な分野であり，従来の経験的，帰納的診断法（pattern method）より更に理論的，客観的診断法が望ましい。かかる意味でGrant^1）はQRSとT波をmean vectorとして表わし，その変化を研究した。このベクトル的解釈法は古くはEinthovenの正三角形理論，更にWilson, Ashman^2），　Bayley^2），Burch^3）等のVentricular Gradientの概念に基ずくもので，現在まで行われている各種誘導曲線は相互に無関係なものではなく，心臓内に起る一つの電気現象即ち心臓ベクトルの各誘導軸への投影と考える。Grantは実用簡便化を目的としloopの代りに主としてmean vectorを用い，胸部を円筒と見做し，胸部誘導の移行帯を応用して得られる空間に於けるMean Spatial QRSとT Vec—torの夾角即ちMean Spatial QRS-T Angleは健常者では一定で，心筋傷害，心室肥大，脚ブロック等に於て，その増大を指摘し，心電図の判定にMean Spatial QRS-T Angleの有意義なことを立証した。これについては私も前に紹介したので^4），その詳細な説明は省略するが，本法の実用・簡便性の特徴の故に米国に於ては急速な普及発展をみた。

The magnitude and direction of the mean spatial QRS and T vectors (SÂ_QRS, SÂ_T) and the mean spatial QRS-T angle {∠(SÂ_QRS･SÂ_T)} were calculated and compared statistically with significant level 0.05 in ten age groups of total 525 normal subjects from newborn to 80 years of age.
