鈍的膝窩動脈損傷に対しcomposite graftによる血行再建を行い救肢し得た1例

A Case of Revascularization with Composite Graft for Popliteal Vascular Blunt Trauma

要旨　症例は22歳女性．仕事中に右膝を重機に挟まれ救急搬送された．CTにて，右膝窩動脈損傷・閉塞，大腿骨遠位端骨折，腓骨近位端複雑骨折，膝関節と周囲に著明な血腫を認めた．早期の血流再開が救肢のために必要と判断し，骨折の修復の前に，右浅大腿動脈と右後脛骨動脈のバイパス術を行った．グラフトには，8～5mmのtaper型のリング付PTFEグラフトと5cmほどの大伏在静脈のcomposite graftを使用した．受傷後の虚血時間は約7時間，術後のmax CKは8,810IU/lであり，減張切開は必要であったが，術後透析は必要としなかった．鈍的外傷に伴う膝窩動脈損傷に対し，人工血管と大伏在静脈のcomposite graftを使用したバイパス術にて24カ月の開存を確認し救肢することができた．本法は膝関節部の高度損傷に合併した血流障害に対して一つの有効な選択肢となると思われた．

　A 22-year-old female was referred to our hospital due to blunt trauma in the right knee joint. Contrast-enhanced computed tomography showed the right popliteal artery was occluded at the point just proximal to the trifurcation because of a complex fracture of the fibula. We performed right superficial femoral artery-posterior tibial artery bypass using a composite graft(8-5mm taper typed polytetrafluoroethylene graft and great saphenous vein)because of revascularization as quickly as possible for limb salvage. The patient could walk with a crutch and the graft was patent on the 24th postoperative month. This procedure may be one of the effective procedures for popliteal vascular blunt trauma.
