要旨●近年，内視鏡機器の進歩により多くの咽頭表在癌が発見されるようになったが，術前深達度診断法は定まっていない．今回，咽頭表在癌に対する経口エコーの深達度診断能について検討を行った．当院で内視鏡的咽喉頭手術を施行した46例51病変に術前経口エコーを施行した結果，上皮下浸潤癌（SEP癌）の正診率82.4%，感度85.2%，特異度79.2%，陽性的中率82.1%，陰性的中率82.6%であった．また，腫瘍厚が1,000μm以上のSEP癌（SEP-deep）に対する経口エコーの診断能の正診率は86.3%，感度は91.7%，特異度は84.6%，陽性的中率は64.7%，陰性的中率は97.1%であり，経口エコーは咽頭表在癌の深達度診断に対して有用であると考えられた．

　Superficial pharyngeal carcinoma has been recently identified due to advancements in endoscopic equipment. However, the methods of diagnosing the depth of pharyngeal cancer remain unclear. This study examined the usefulness of US（ultrasonography）in diagnosing the depth of superficial pharyngeal cancer in 46 patients with 51 lesions who underwent both ME-NBI（magnifying endoscopies with narrow-band imaging）and US. The accuracy（82.4%）, sensitivity（85.2%）, positive predictive value（82.1%）, and negative predictive value（82.6%）rates of US for SEP（subepithelial propria）were higher than those of ME-NBI and macroscopic classification, whereas that for SEP with a tumor thickness of ≥1000μm were 86.3%, 91.7%, 84.6%, 64.7%, and 97.1%, respectively. In conclusion, US was suggested to be useful in diagnosing the depth of superficial pharyngeal cancer.
