非乳頭部十二指腸上皮性腫瘍に対する超拡大内視鏡診断（endocytoscopy）の有用性

The Utility of Endocytoscopy in Diagnosing Superficial Non-ampullary Duodenal Epithelial Tumor

要旨●非乳頭部十二指腸腺腫・癌の統一された内視鏡的な診断基準はいまだない．加えて，十二指腸は生検の診断精度と，生検による治療時の難度の上昇が課題である．超拡大内視鏡観察で術前診断ができれば組織学的侵襲がないために意義は大きいと考えられる．自験例における超拡大内視鏡観察の術前診断精度を検討したところ，その正診率は89.6%であった．本稿では非乳頭部十二指腸上皮性腫瘍に対する超拡大内視鏡診断の現状を記す．

　Endoscopic diagnostic criteria for SNADETs（superficial non- ampullary duodenal epithelial tumors）have not been established. Additionally, the diagnostic accuracy of biopsy and increased difficulty in providing biopsy-based treatment in the duodenum are major challenges. Preoperative diagnosis using endocytoscopy would be of great importance as there is no histologic invasion. In this study, the accuracy of preoperative diagnosis made using endocytoscopy was investigated, and the correct diagnosis rate was 89.6%. This article describes the current status of the endocytoscopic diagnosis of SNADETs.
