要旨●CTC（CT colonography）は，大腸に特化したCTの呼称であり，肛門からの送気により大腸を拡張させた状態で撮像する手法である．本邦では大腸癌の術前検査，大腸癌やポリープのスクリーニング検査として普及している．大腸癌術前検査においては，CTでのリンパ節転移診断，遠隔転移診断に加え，CTCによる局在診断および深達度診断が可能である．大腸ポリープに対するスクリーニングとしての成績は6mm以上のポリープで感度73〜98%，特異度80〜93%と十分に許容しうるものである．また，近年CT撮像機器の技術革新や人工知能（AI）の導入により，CTCは大腸癌診療において新たな局面を迎えている．

　CTC（CT colonography）is a CT technique specialized for the large bowel, in which the latter is dilated with air supplied through the anus. In Japan, it is widely accepted as a preoperative examination for CRC（colorectal cancers）and as a screening test for colorectal polyps. The primary tumor can be visualized in the preoperative workup to examine nodal and distant metastases. With screening tests for colorectal polyps, the detection rate is acceptable with a sensitivity of 73%-98% and specificity of 80%-93% with sizes of 6mm or greater. Currently, CTC has entered a new phase in CRC treatment thanks to technological innovations in CT equipment and advances in artificial intelligence.
