 NET(neuroendocrine tumor)is a lesion with low malignancy and good prognosis, whereas NEC(neuroendocrine carcinoma), a rare subtype of gastric cancer(accounting for 0.2%-0.6% of all gastric cancers), exhibits aggressive growth and has a high incidence of lymph node metastasis. As most NECs are diagnosed in the advanced stage and effective treatment for these lesions has not yet been established, the prognosis remains poor. NET is a more indolent tumor than NEC and appears as a small reddish or isochromatic elevated lesion. Dilated vessels are frequently observed on the surface. The most common macroscopic type of advanced NEC is the localized ulcerated type. The superficial lesions display a depressed type or ulcerated type with submucosal tumor-like marginal elevation. The magnifying narrow-band imaging reveals an irregular MV(microvessel)pattern plus an absent microsurface pattern as the characteristic finding, and the irregular MV pattern is scarcely present, with a poor meandering and non-network form. Because of the low diagnostic accuracy of NEC by biopsy specimens using hematoxylin and eosin staining, immunohistochemical staining is strongly recommended for lesions endoscopically suspected as NEC.
