要旨●患者は83歳，男性．便潜血陽性にて下部消化管内視鏡検査を施行したところ，横行結腸に3cm大の粘膜下腫瘍を認めた．くびれを有し，表面はやや凹凸のある比較的軟らかな粘膜下腫瘍であり，表面に小潰瘍を伴っていた．生検にて診断に至らず，外科的切除術を施行した．病理組織学的には好酸性細胞質と円形の核を有する円形細胞の充実性増殖を示しており，免疫組織化学的検索および遺伝子検査により，横行結腸に発生したPEComaと診断した．

　An 83-year-old male with fecal occult blood underwent colonoscopy and was found to have a submucosal tumor（diameter, 3cm）in the transverse colon. The submucosal tumor was relatively soft, formed a constriction, and had a somewhat irregular surface along with a small ulcer. Surgical resection was performed without biopsy-based diagnosis. Histopathological analysis revealed solid proliferation of round cells with eosinophilic cytoplasm. Findings of immunohistochemical and genetic analyses established the diagnosis of perivascular epithelioid cell tumor of the transverse colon.
