 GAFG(gastric adenocarcinoma of fundic gland type)is a newly added special type of cancer documented in the 15th edition of the Japanese classification of gastric carcinoma. GAFG, an uncommon variant of gastric adenocarcinoma, has distinct clinicopathological, immunohistochemical, and endoscopic features. GAFG is defined by positive immunohistochemical staining for pepsinogen I(chief cell marker)and/or HK-ATPase(parietal cell marker)and is not associated with Helicobacter pylori infection. Histopathologically, GAFG is classified into pure GAFGM, which exhibits differentiation toward the gastric foveolar epithelium in addition to fundic gland differentiation. However, we need to establish a new detailed GAFG classification based on atypia and cell differentiation because of the existence of other variations of GAFG. Additionally, further investigation concerning the molecular and biological features and establishment of clinical guidelines on GAFG is necessary to elucidate the natural history of GAFG.
