Crohn病の内視鏡的重症度評価─小腸病変の評価法:内視鏡とほかのモダリティの比較

Small Bowel Endoscopic Evaluation in Patients with Crohn's Disease

要旨●Crohn病において小腸病変評価は治療方針を決めるうえで重要である．バルーン内視鏡の普及により小腸病変を詳細に評価できるようになった．定量的な内視鏡評価は重要であるが，現在小腸を定量的に評価可能な内視鏡スコアは存在しない．本稿では当施設での内視鏡評価方法および内視鏡スコアについて紹介する．またバルーン内視鏡検査は臨床的に非常に有用である一方，検査可能な術者や施設が限られる点などを考慮すると，内視鏡の代用となりうる検査についても重要な課題であり，当施設で行っているバルーン内視鏡とMRI検査との直接比較についても報告する．今後，バルーン内視鏡検査の利点，欠点をよく検討し，その他の検査も含めた診療のストラテジーを構築する必要がある．

　Evaluation of small bowel lesions in Crohn's disease is essential for determining the optimal treatment. With the increasing popularity of BAE（balloon-assisted enteroscopy）has facilitated the detailed evaluation of small bowel lesions. Quantitative endoscopy evaluations are important, but currently, there is no endoscopic score system to assess the small bowel ; thus, we developed an endoscopic evaluation method and endoscopic score system at our hospital. BAE is suitable for use in clinical practice, but considering the possibility of other operators and facilities, it is necessary to develop other examination procedures as well. We reported our findings from direct comparison of BAE and MRI. It is necessary to carefully study the advantages and disadvantages of BAE and construct strategies for medical treatment, including other examinations.
