要旨●hot biopsy・polypectomy・EMRは高周波発生装置を使用した，いわゆる“焼き切る”方法で大腸病変を切除する治療法である．内視鏡的切除において重要なことは再発することなく治療することであり，それは小・微小病変に対しても同様と考える．高周波発生装置を使用し“焼き切る”治療法は切除断端に熱変性を生じ，病変が切除辺縁に近接している場合でも完全切除率の向上が期待できる．内視鏡治療において注意すべき偶発症は主に出血・穿孔であるが，基本的な手技を段階ごとに確認しつつ確実に施行することにより，偶発症の予防につながる．また，偶発症が発生したときは安全を第一に考え，迅速な対応が必要である．

　Hot biopsy, polypectomy, and EMR are procedures for treating colorectal lesions using high-frequency generators. While performing endoscopic resection, it is important to treat colorectal lesions, as well as small colorectal lesions, so as to prevent recurrence. These procedures that use high-frequency generators cause a burning effect at the resected end, which is expected to improve the complete resection rate.

　Major complications of endoscopic procedures such as bleeding and perforation may be prevented by efficiently performing the basic procedures. If complications are suspected, it is important to first consider patients' safety and respond promptly.
