虫垂病変の臨床―画像診断―腹部エコー診断の立場から

Sonographic Diagnosis of Bowel Ischemia

要旨　虫垂の体外式超音波（以下，US）診断について概説した．虫垂の確実な描出がUS診断の第一歩であるが，そのためには，上行結腸―回盲弁―虫垂開口部の順に同定を進める系統的走査が必須である．急性虫垂炎は虫垂の腫大，層構造の改変，同部直上での最大圧痛，さらには虫垂間膜や回盲部の浮腫などにより，その重症度も含めて診断可能である．急性虫垂炎に類似した病態であり，一般的に鑑別が困難とされる虫垂憩室炎もその特徴的なUS像から診断は容易である．一方，腫瘍性疾患においては，多くの場合穿孔などの合併症の出現が受診契機であることから，US施行時に必ずしも典型的な画像を呈しているとは限らないため注意を要する．基本的には他の消化管における腫瘍性疾患のUS像と同様の画像を呈するが，虫垂粘液囊腫は開口部が狭く盲端に終わる虫垂の特性上，内腔にほぼ無エコーな粘液が貯留して全体に囊胞状の形態を呈する点で特徴的である．また，造影USは壊疽性虫垂炎の確定診断や虫垂粘液囊腫における充実成分と粘液の鑑別などに非常に有用である．炎症，腫瘍いずれの病態においても虫垂の層構造を含めた詳細な評価が可能であるUSは，その形態学的診断上第一選択に位置付けられるべき検査法である．

　Ultrasonography（US）is a useful technique for diagnosis of appendiceal diseases. Correct and constant identification of the appendix is the essential feature of US diagnosis. For this purpose, the systemic scanning technique, which consist of stepped visualization of the ascending colon, ileocecal valve, and appendix orifice, must be used. Once the appendix is visualized, US diagnosis of acute appendicitis is not easily confused with other diagnoses, such as appendiceal swelling, submucosal edema or loss of stratification, and ileocecal and/or mesoappendceal edema. In addition, contrast ultrasound is often useful for the diagnosis of gangrenous appendicitis. The basic features of neoplastic lesions of the appendix does not differ much from those of other parts of the gastrointestinal tract except for the appendiceal mucoceles which present cystic features similar to those of the appendix. However, the US features of other tumors, such as carcinoids or lymphomas, are often modified by accompanying inflammation at the time of detection ; thus they should be carefully diagnosed.
