Diagnosis of Inflammatory Bowel Disease with Ultrasound and Computed Tomography

炎症性腸疾患（IBD）の診療において，超音波やCTは，単に内視鏡やX線造影の代替法にとどまらず，貫壁性，さらには壁外の変化も把握することが可能なことから，その初期診断，活動性評価，合併症の診断などに有用である．比較的新しい技術としては，造影超音波による微細血流の評価，エラストグラフィーによる病変の硬度の評価などが挙げられ，CTではわかりやすい表示法としてのCT colonography（CTC）などが注目されている．IBDの治療法が進歩し，治療に対する概念も変化しつつある中で，病変局所の活動性評価は従来に増して重要となっているが，頻回に経過観察を要する本疾患においては，今後超音波やCTなどの苦痛の少ない検査法の活用が期待される．

　In this chapter, the clinical usefulness of CT and ultrasound in the clinical management of IBD（inflammatory bowel disease）is discussed. Both modalities enable us to evaluate transmural and also extramural changes of the affected bowel segment in patients with IBD, which is useful for not only disease screening but also for evaluating disease activity and detecting complications such as fistula. Basically the wall becomes thicker as the lesion becomes more active, while some exceptions exist especially in cases with Crohn's disease. Contrast ultrasound, elastography, and CT enterocolonoscopy are the new methods considered to be of great use in daily practice. CT and ultrasound will play a more important role as non-painful methods for assessment of focal disease activity, and will lead to changes in the therapeutic strategies and methods for IBD patients.
