大腸の新しい超音波診断―造影超音波検査

Contrast Ultrasound for the Diagnosis of Colonic Diseases

要旨　本邦で使用可能な造影剤であるLevovist^TMおよびSonazoid^TMを用いた大腸の造影超音波に関して概説した．Levovist^TMを使用する場合は間欠送信によりマイクロバブルを破壊して信号を得るflash echo imagingを用いることが多く，またSonazoid^TMの場合は低音圧ハーモニックイメージングを用いてリアルタイムに観察することが多い．これらの手法により簡便かつ非侵襲的に大腸壁の微細循環を描出することが可能であり，腫瘍性疾患においてはその鑑別や悪性度評価，炎症性疾患では活動性の評価，さらに消化管出血や虚血の評価に応用が可能であると考えられ，日常臨床における高い有用性が期待される．

　Contrast ultrasound of the colon is described. Under the injection of Levovist^TM, intermittent scanning called“flash echo imaging”is necessary to evaluate the microperfusion of colonic lesions, while real-time visualization is possible with low mechanical index harmonic imaging under the injection of Sonazoid^TM. With these techniques, we can evaluate the vascular structure and vascularity of lesions, which helps us assess the malignant potentials of neoplastic lesions, disease activity of inflammatory disorders, and diagnose hemorrhage and ischemia. This novel method, being non-invasive and easy, is thought to be very useful in daily practice.
