超音波内視鏡による早期胃癌深達度診断

Thin Probe Endoscopic Ultrasonography for Pretherapeutic Depth Staging for Early Gastric Cancer

要旨　生検結果が分化型癌・内視鏡的UL（－）の早期胃癌病変において，EUSでcT1a（EUS-MあるいはEUS-M/SM境界領域，すなわち第3層の変化がないか，あっても深さ1mm未満）であることは，内視鏡的切除の結果が絶対適応，あるいは適応拡大の範囲内であることを，9割以上の確率で推定していた．誤りの要因の多くは，未分化型混在癌の存在であった．UL（＋）病変の深達度診断については，現時点ではEUSの有用性を確認できなかった．未分化型癌病変におけるEUSでの確実な粘膜内病変の同定には課題が残っている．

　Thin probe EUS is suitable for cT1a staging of early gastric cancer lesions with differentiated type histology in biopsy without endoscopic ulcerous change. However, mixed intestinal-and diffuse-type histology runs a risk of under staging in EUS. Early gastric cancer lesions with endoscopic ulcerous change and diffuse type histology in biopsy cause some difficulties in EUS depth staging.
