頻回の下血を主訴として入院した患者に,X線学的に空腸の腫瘍を発見し,その組織像がStout1)の提唱するleiomyoblastomaであった症例を経験した.
核の周囲が明るく,胞体が好酸性を呈する円形細胞より成るこのleiomyoblastomaは比較的稀な疾患であり,胃に発生することが最も多いが,食道,腸などの他の消化管,子宮,後腹膜などにも発生する.
A case of Leiomyoblastoma in the jejunum is reported. A 76-year-old woman complained of massive melena. On X-ray examination, a filling defect of hen-egg size was demonstrated in the jejunum about 40 cm distal to the Treitz's ligament. Operation was performed because melena persisted and malignancy could not be denied.
The resected specimen showed a polypoid intramural tumor measuring 5.0×4.0 cm and partly covered with blood coagula. Microscopically, the tumor chiefly consisted of round cells with clear and acidophilic cytoplasm and round to oval nuclei and vascular stroma. There were no mitotic figures in all step-sections the whole tumor. The vascularity was also confirmed by microangiography in the resected specimen.
Leiomyoblastoma is a rare disease and yet 37 cases of that the stomach have already been reported in Japan. We could not find, however, any case-report of that of the small intestine in Japan.
