Acanthosis nigricans(以下A. n.と略す)は臨床上特有の像を示し診断は容易であるが,その症例の半数に悪性腫瘍を合併するため注目されている1)~7)13).最近自覚症なく本症の診断によりすでに進行した胃癌を発見したが,不幸な転帰をとった1例を経験したので報告し,併せて最近10年間の報告例について文献的考察を加えた.
A 66 years old man was recognized to have hyperpigmentation on his neck, mamilla areas and his face since one month before. The pigmentation gradually grew dark and spread all over the body except his palms and soles. During the period he felt no subjective gastric complaints.
By radiographic examination we have discovered advanced gastric cancer (remarkably advanced lesion).
Histopathologically, skin lesion (inguinal area) has shown typical acanthosis nigricans (that is, hyperkeratosis, papillomatosis and hyperpigmentation on the epidermis).
The preoperative diagnosis advanced infiltrating gastric adenocarcinoma without metastasis.
We noticed metastases in the left hepatic lobe and surrounding lymphnodes and performed subtotal gastrectomy and resection of affected lymphnodes.
After the operation temporally and clinically the skin color became lighter than before operation.
We have referred to the literature of the rescent reports for the past 10 years.
