食道癌および噴門癌術後の消化吸収

Observations on Intestinal Absorption after Surgical Operations for Esophageal and Upper Gastric Cancer

　食道癌および噴門癌手術に伴う．上部消化管の欠損，消化吸収面積の曠置，脱落による直接的影響は致し方ないものとしても再建臓器，再建コースの工夫によって，どの程度まで術後の消化吸収を生理的条件に近く維持できるのかといった，ひとつの懸案がある．このような目的で通過時間の延長，嚥下食物の十二指腸通過を計る代用胃再建術式が工夫されている．最近2年間に術前後の成績に関して調査しえた30例を中心に再建コース別によって，十二指腸を通過するBillroth Ⅰ法型の場合と直接食道より空腸へ移行するⅡ法型との間にどのような優劣がみられるのか，truncal vagotomyはどの程度消化吸収に影響してくるのか，また，再建胃管の機能や術後の膵機能はどうなっているのか，また，それらが術後消化吸収にどの程度関与しているのであろうかといった諸問題につき述べてみたい．また，MCT投与の価値についても言及してみたい．

　We have examined postoperative intestinal absorption and digestion in esophageal and upper gastric cancer with some discussions on this problem.

　The grade of postoperative malabsorption is severe on lipids and slight on glucocides and protein. Moreover, the digestive course via the duodenum is better than that from the esophagus to the jejunum. We assumed that asynchronism of the diet with the digestive secretions may chiefly account for this mechanism. Vagotony also promotes these conditions.

　The secretion from the reconstructed stomach or residual stomach remarkably decreases, while post-operative functions of the pancreas are not lowered except enzyme activity. For postoperative steatorrhea we were able to demonstrate superiority of MCT diet. Influence of the course of organ reconstruction and vagotomy should not be neglected as well.
