 The patient, a 55 years oId man, was examined at Chiba Cancer Center Hospital for the complaint of constipation. The barium enema examination showed a small polypoid lesion with granular surface in the rectum. The endoscopic examination disclosed a flat elevation which had granular surface with bleeding and necrotic coat. Biopsied specimen at the first time revealed atypism without any definite finding of carcinoma. We followed up this patient for one year and ten months. During this period, the patient had barium enema examinations eight times and endoscopic ones and biopsies six times. Biopsied specimens of the last two examinations revealed adenocarcinoma papillare. So the patient underwent an operation by pull through method, avoiding anus preternaturalis. Resected specimen showed a small flat elevation, Ⅱa type,7×5 mm in diameters. Histologically the elevation was diagnosed adinocarcinoma papillare.

 The points of interest in this case in our opinion are the foliowing.: -

1. Radiographs showed no change of the size and shape of the elevation for about one year and ten months.

2. Endoscopic findings did not change in size and shape, but sometimes bleeding and necrotic coat were seen. It meams only the lesions surface was changeable in the course of time.

3. The first biopsied specimen, diagnosed atypism at that time, was reviewed carefully and also diagnosed as carcinoma lastly.
