はじめに
全く愁訴なく,対がん協会の胃集検で発見された,Ⅱa+Ⅱc様の形を呈した早期胃癌の1例を報告する(この症例は,昭和44年4月4日,仙台市で行なわれた早期胃癌研究会の席上検討された).
This is a case report of an early gastric cancer, suggesting Ⅱa+Ⅱc type lesion, found at a gastric mass survey and later operated on in the Miyagi Prefectural Center for Adults' Diseases.
A 25-year-old female, a company employee, with no subjective symptoms to complain of, was nevertheless found to have a fleck at the gastric angle and deformity of the antrum in indirect x-ray pictures, so that he was referred to the authors' department for thorough check-up.
In the pictures of both x-ray and endoscopy, a finding suggestive of Ⅱa+Ⅱc cancer was observed on the greater curvature of the antrum slightly near the anterior wall.
Cancer cell was positive in brushing cytological diagnosis under direct view.
In the resected specimen, an oblong Ⅱc-like lesion, measuring 13 by 2mm, shaped as of a gourd, was seen on the lesser curgature of the antrum a little anteriorly. Surrounding protuberances seemed to have been formed by the rugae.
Histologically it was adenocarcinoma tubulare, with marked rejuverated lymph follicles in its neighborhood. Its infiltration was localized within the submucosal layer. Cancer cells were found only in the area suspected as of Ⅱc, and what had been regarded as Ⅱa was nothing but the outcome of mucosal elevation from underneath caused by cancer cells and aggregate of lymphfollicles.
