  • Look Inside

Tremendous development has taken place in the field of fiberscope, and not only the stomach but also the duodenum is going to be exploited. In this paper is described a method of observing the ampulla of Vater by inserting fiberscope down into the pars descendens of the duodenum. It shall be aimed at in the future to visualize by contrast medium the choledochous and the pancreatic ducts through cannulation of the ampulla of Vater. If this becomes possible, it will greatly contribute to the diagnosis of diseases of the liver and the pancreas.

For the study of the duodenum, fibergastroscopy for biopsy seems to be the best tool, and the authors have succeeded in inserting it into the duodenum. Under general anesthesia and supervised by TV fiuoroscopy, fiberscope is inserted through the pyloric ring down into the upper part of the duodenum. Contrast medium is then let in through the tip of fiberscope. After visualizing this region, it is then placed downward into the pars descendens. This method has been successfully employed in seven out of ten patients. Feasibility of this maneuver seems to depend on the configuration of the duodenum from the pyloric ring downward.

Observation of the ampulla of Vater is impracticable by gastrofiberscope for biosy, but it is possible if S type fiberscope (for observation) is used.

A sine qua non for the success of duodenal fiberscope is discussed in this paper.
