胃ポリープ経過観察中に発見されたⅡb型早期胃癌の1例

IIb Type Early Gastric Carcinoma Diag-nosed during Follow-up of Coexistent Gastric Polyp, Report of a Case

はじめに

　胃ポリープの経過観察中に発見された純粋Ⅱb型早期胃癌の1症例を経験したので報告する．

　A 63-year-old woman known to have a gastric polyp in the antrum of the stomach was incidentally found to have a IIb type of early gastric carcinoma at an annual endoscopic follow-up.

　An endoscopic view with a small amount of air insufflation revealed an abnormal area of mucosal redness on the posterior wall of the upper body of the stomach. With a moderate amount of air insufflation, the lesion looked slightly depressed and somewhat granular. Further lnsufflation of air made the lesion discolorated and obscured the margin of the lesion.

　Barium meal examination merely showed an area of abnormal barium coating.

　On the resected specimen, the margin of cancerous area could not be pointed out visually. The exact extent of the Iesion was learned by microscopic examination of serial sections of the resected specimen.

No recognizable elevation or depression of the lesion from surrounding mucosa was noted.
