胃生検組織診断分類改正案への対応―臨床の立場から

Revised Group Classification of Gastric Biopsy Seen from a Clinical Standpoint

要旨　胃癌診断のための胃生検Group分類が1969年に作られて10年以上経過し，1983年に改正案が作られたが，改正点について臨床的立場から検討した．Group分類の中でGroupⅢの内容が，隆起型異型上皮のみでなく，陥凹型異型上皮をも含めたものであって，特にGroupⅢと判定された陥凹性病変では，隆起性病変の生検診断のごとく比較的単純でなく，癌の可能性を考慮して，慎重なる態度が望ましい．

　Group classification of gastric biopsy for diagnosis of gastric cancer was revised in 1983 about ten years after its first establishment in 1969. Authors evaluat-ed the revised one from a clinical standpoint. Lesions of Group Ⅲ in the group classification contain not only atypical epithelium of elevated type but also that of the depressed type. Especially when a depressed type lesion is diagnosed as Group Ⅲ, it is not so easy to diagnose it benign or malignant as in a lesion of the elevated type. Therefore, careful attention must be paid to these types of lesions because of their cancer possibility.
