 A 32 year-old man was admitted with the chief complaint of abdominal pain. After a temporary remission, he suffered from the colicky attack accompanied by vomiting on the 17th day and was laparotomized under the diagnosis of the obstruction of the small intestine. A five-fold cylindrical invagination enterica was found in the ileal lumen, 120cm from the terminal portion, with a protruding thumb's headsized lesion as the top, locating at the antimesenteric area of the small intestine. Thereupon, the resection about 25 cm long was made of the small intestine including the polypoid lesion. On gross observation and histological examination, the elevated lesion was diagnosed as the inverted Meckel's diverticulum. Further, the heterotopic pancreatic tissue was demonstrated, invested in the fatty tissues in the subserosal layer of the diverticulum, containing the Langerhans islet tissue and the exocrine tissue of the pancreas and even tre pyloric gland-like duct.
