進行胃癌の悪性サイクル

Malignant Cycle of the Advanced Cancer

　胃癌に於いては，癌巣内の潰瘍でも良性潰瘍と同様に縮小・治療・再発をくりかえすことのあることは，欧米では，すでに古くから知られていたが，当時はあくまでも，きわめてまれな，めずらしい現象として理解されていたようである．

　また，わが国に於いても，潰瘍縮小の観察された胃癌例を筆者らが「悪性潰瘍の表面変化」と題して報告を行なった昭和40年4月頃までは，かかる現象も，非常にまれな奇異なできごととしてしか受けとられていなかったようである．しかしながら近年わが国に於ける胃X線ならびに胃内視鏡検査の著しい普及により，次第にこのような症例が数多く，しかも明瞭に観察されるようになって来た．癌巣内潰瘍でも縮小，瘢痕化することがあるというこの事実は，われわれ臨床医にとっては，胃の潰瘍性病変の診断にあたって潰瘍が縮小したからといって決して油断してはならないという診断上の警告として受け入れられたが，いっぽう，当時潰瘍癌の判定規準をめぐって論争中の病理学者の間では特に潰瘍底の再生粘膜の解釈に非常に重要な情報を提供し，大きな波紋を投げかけたことは周知のごとくである．そして昭和40年村上は胃癌病巣内潰瘍の，このような現象を良性潰瘍に於けるライフサイクルに対応して悪性サイクルと呼ぶことを提唱したが，以来悪性サイクルに関する報告は数多くみられ，今日に於いては陥凹性早期胃癌で悪性サイクルの認められることは常識として考えられるようになっている．

　Clinical and histopathological studies have been retrospectively made on 39 cases of advanced gastric cancer encountered up to the end of 1970, including 12 showing reduction of concomitant ulcer. The results obtained as are as follows:

　1. All advanced gastric cancer displaying malignant cycle were either of Ⅱc＋Ⅲ or Ⅲ＋Ⅱc subtype, simulating in many respects early cancer. No typical advanced cancer that comes under Borrmann's classification was observed.

　2. The presence or absence of ulcer reduction in advanced carcinoma possibly depends more on the density of cancer tissue in the ulcer floor than on the depth of cancer invasion.

　3. Great difference was seen in the developmental process of cancer between Ⅱc＋Ⅲ or Ⅲ＋Ⅱc and those typical advanced cancers that fall under Borrmann's classification.

　4. Regenerated epithelia possibly play different role in the course of ulcer reduction between undifferentiated adenocarcinoma and well differentiated one.
