 Medical management of gastric ulcer is now undergoing change of principle from the former suppression of aggressive factors to intensifying mucosal defensive factors. Nevertheless, no definite method of treatment for it has been established so far. In discussing the cure of gastric ulcer it is necessary not only to fully grasp the salient features of healing tendency inherent in it but also to know thoroughly its long-term prognosis. For this purpose we have described here several urgent problems regarding the courses of gastric ulcer on the basis of our experienced cases. The prognosis of gastric ulcer in 306 cases 1~2 years after the onset of the disease was favorable in 43 per cent, unfavorable in 34 per cent, and the remaining 23 per cent was unknown. The factors that most often worsen the prognosis included the age of patients-men 40 to 50 years of age, ulcer at the gastric angle more than 2 cm in length, MAO measuring more than 25 mEq/hr and previous history of peptic ulcer. The percentage of recurrence or exacerbation of ulcer was higher in spring than in other seasons. Previous medication had nothing to do with either of it. For these points reference has been made to the literature.

 Whether one should have recourse solely to medical treatment or to surgical intervention should be determined in individual cases, and no uniform criteria for treatment are to be established.
