 Hamartomatous inverted polyp is a rare disease that was first described by Allen in 1966.

 A 34 year-old man was admitted to Nagoya Ekisaikai Hospital on November 5, 1985, for complaints of diarrhea and rectal bleeding.

 On barium enema two elevated lesions were found in the rectum (Fig. 1). Endoscopy confirmed these two lesions located along the longitudinal axis of the rectum and covered with normal appearing mucosa (Fig. 2, 3).

 Endoscopic ultrasonography revealed many cysts in the submucosa (Fig. 4).

 Local excision was performed. Microscopically, many cysts covered with columnar epithelium without malignant transformation were seen in the submucosa. The lamina propria was occupied by excessive amount of collagen fibers and smooth muscle cells showing fibromuscular obliteration (Fig. 6a, b). Pathologic study revealed colitis cystica profunda.

 Young age, no previous history of inflammatory bowel disease, and the lack of inflammatory reaction around the lesions as well as the histological findings as above formed the basis of diagnosing this case as hamartomatous inverted polyps.
