 A patient,50 year-old woman, was admitted to our hospital with chief complaints of epigastralgia and fever. Echogram and computerized tomography showed a choledochal stone and intrahepatic stones. ERCP demonstrated a stenotic site of the left intrahepatic duct and intrahepatic calculi in its distal branches. After transhepatic cholangioscopic lithotomy, left lobe was resected. Since at laparotomy biliary muds and suppurative bile were observed in the intrahepatic duct of right caudate lobe, its partial resection was also performed.

 Postoperatively she is in good health.
