胃原発の胃カンジダ症の1例

Gastric Candidasis, Report of a Case

要旨　患者は76歳，女性．食欲不振を主訴として来院．上部消化管造影で胃石様の可動性のある隆起性病変と，胃体上部に散在あるいは集簇した小豆大の小隆起性病変を認めた．内視鏡検査による観察ではこの可動性のある隆起は黄白色，酒粕様，脆弱な集塊物であった．また，胃体部の小隆起は地図状あるいは塊状となった黄白色付着物であった．この付着物および酒粕様集塊の培養でCandida albicans Aの発育が認められ胃カンジダ症と診断した．抗真菌剤の投与により自覚症状は消失し，消化管造影での異常所見も消失した．本例は重篤な合併症はなく，抗生剤，ステロイド剤などの使用の既往もなく胃カンジダ症発生要因の面で興味ある症例と思われた．

　A case of gastric candidasis is presented. A 76-year-old woman was admitted to our hospital complaining of loss of appetite. X-ray examination showed a bazor-like mass with the radiopacity of soft tissue in the stomach and cobblestone-like appearance, on the posterior wall of the body of the stomach.

　Gastrofiberscopy revealed multiple yellow-white plaque-like lesions on the mucosa of the body of the stomach. Candida albicans A was obtained by culture of the biopsy specimen. Her symptom improved after treatment with nystatin.

　A follow-up examination showed that the cobblestone-like appearance had vanished.
