要旨 〔症例1〕は,70歳,女性.胃癌術後の検診目的で来院し,X線検査で胸部上部食道に棍棒状の隆起性病変を指摘された.〔症例2〕は,52歳,女性.嚥下時不快感を主訴に来院し,X線検査で胸部中部食道に亜有茎性隆起を指摘された.いずれの症例も内視鏡検査では頂部に白苔の付着した発赤調の表面平滑な隆起であり,生検でpyogenic granulomaと診断された.本症はポリープ状の易出血性腫瘤で,外傷を受けやすい皮膚や口腔粘膜に発生する良性肉芽腫性病変である.口腔粘膜以外の消化管での発生は極めてまれであり文献的考察を加えて報告した.
〔Case 1〕 was a 70-year-old woman who visited our hospital for a follow-up study after partial gastrectomy due to gastric cancer. The upper gastrointestinal radiography revealed polypoid lesions resembling a club in the upper portion of the esophagus.
〔Case 2〕 was a 52-year-old woman who visited our hospital with discomfort in swallowing. Upper gastrointestinal radiography revealed a lesion in the middle portion of the esophagus. Endoscopic examination revealed a polypoid lesion which was reddish in color and had a white coating on the surface mucosa. Pathological examination of the biopsy specimens revealed pyogenic granuloma. Pyogenic granuloma is a benign granulomatous lesion with a hemorrhagic tendency which occurs in the skin and oral mucosa associated with trauma. The ocurrence of this lesion in the gastrointestinal tract apart from the oral mucosa is very rare. We reported two cases of pyogenic granuloma in the esophagus.
