4．大動脈内超音波による食道癌直接浸潤の診断

Intraaortic Ultrasonography for the Diagnosis of the Aortic Invasion in Advanced Esophageal Cancer

要旨　進行食道癌はしばしば大動脈に浸潤し，正確な診断がその後の治療を決定する上で重要である．われわれは大動脈内にアロカ社製の20MHzの回転型8F細径プローブを挿入し，大動脈内より壁浸潤の有無を診断した．大動脈壁は外膜，中膜，内膜が3層（high，low，high）に描出される．腫瘍はlow echoic massに描出され，外膜のhigh echo lineが途切れていれば浸潤ありと判定する．しかし問題になるのは中膜浸潤であり，low echoic lineがmassに置き換わっている場合は手術適応なしと判定される．最近は三次元USを用い，浸潤の判定をより正確に行うことを目指している．

　Intraaortic ultrasonography (IAUS) offers both a direct and exact diagnosis of the extent of invasion. When intima and media can be identified by IAUS, tumor resection is possible. IAUS is a safe and reliable diagnostic tool for determining the resectability of locally advanced esophageal cancer. Recently, 3D IAUS was tested for the diagnosis of invasion. We expect this new technique will provide more precise information for the diagnosis of aortic invasion.
