 To clarify the risk factor for lymph node metastasis in submucosally invasive carcinomas of the large intestine, the associations between adverse outcome (lymph node metastasis or local recurrence) after endoscopic polypectomy alone or followed by surgery for 49 submucosally invasive carcinomas and various clinicopathological parameters were examined using logistic regression analysis. The classification by the level of submucosal invasion (modified Haggitt's classification) alone was strongly associated with adverse outcome (p=0.0381, Odds ratio 11.26, 95% confidence interval 1.14~111.04). Radical surgery should be considered for submucosally invasive carcinoma with level 3 invasion. Further study is needed to select effectively high risk patients with level 2 invasion.
