スキルス胃癌と鑑別を要する非腫瘍性疾患―Cronkhite-Canada症候群

Cronkhite-Canada Syndrome

要旨　Cronkhite-Canada症候群は消化管ポリポーシスに加え，多彩な外胚葉系の消化管外病変を伴う非遺伝性疾患である．ポリポーシスは胃と大腸に好発し，組織像は若年性ポリープに類似する．強い発赤と浮腫を伴った無茎性のポリープが多く，密集して認められる．ポリープ間の介在粘膜にも炎症が存在し，発赤，浮腫，びらんなどの変化がみられ，胃においては巨大皺襞を呈することがあるため，スキルス胃癌との鑑別が必要となる．

　Cronkhite-Canada syndrome is a noninherited gastrointestinal polyposis accompanied with various lesions in ectodermal systems other than the gastrointestinal tract. Polyposis is commonly detected in the stomach and colon, and has histopathological features similar to those of juvenile polyps. Clustered developing edematous nonpedunculated polyps with redness are generally observed. Redness, edema, and erosions are often observed in the mucosa between polyps, and differential diagnosis from scirrhous gastric cancer is important because it is occasionally seen as an enlarged gastric fold.
