腹部超音波検査が診断に有用であった空腸gastrointestinal stromal tumorの1例

Diagnostic Results of Extracorporeal Ultrasonography in a Patient with Gastrointestinal Stromal Tumor of the Jejunum

要旨　症例は60歳，女性．慢性関節リウマチのため当院に通院治療中であったが，2003年7月より貧血と黒色便が出現し入院となった．上・下部消化管内視鏡検査で異常は認めなかったが，腹部超音波検査で左上腹部に小腸壁と連続する約3cmの雪だるま型の充実性腫瘤を指摘された．小腸X線検査では上部空腸にニッシェを伴う約3×2cmの円形陰影欠損を認め，二重造影では立ち上がりのなだらかな隆起として描出され，腸管外にX線透過性の低下した腫瘤陰影を伴っていた．小腸内視鏡検査では中心に深い潰瘍を形成した粘膜下腫瘍として観察された．以上より空腸間葉系腫瘍を疑い腹腔鏡補助下に空腸部分切除を施行した．組織学的には不規則に索状配列した紡錘形細胞がみられ，核分裂像は0個/50HPFであった．免疫組織化学的染色はKIT(＋)，α-SMA(＋)，S-100(－)で平滑筋への分化を示すgastrointestinal stromal tumorと診断した．

　A 60-year-old woman was admitted to our hospital because of rectal bleeding and severe anemia requiring blood transfusion. Abdominal ultrasonography revealed a hypoechoic mass connected with the small intestinal wall, measuring3×2cm in size. X-ray and enteroscopic examinations revealed a submucosal tumor with ulcerations in the jejunum. Partial resection of the jejunum was then carried out. Histopathological examination demonstrated that the tumor was composed of spindle cells without any mitosis. Immunohistochemical analyses showed the tumor cells to be negative for CD34and S-100protein, but positive for KIT and α-smooth muscle actin. Relying on this data, we made a diagnosis of jejunal low grade malignancy of gastrointestinal stromal tumor.
