 Detailed examinations will be performed in our institution for esophageal submucosal tumors (SMT) more than of 20 mm in diameter to decide whether treatment or follow-up is the best decision. The best indication for endoscopic treatment of esophageal SMT is when the tumor is benign or when it is limited to the submucosal layer. However it is not indicated when the tumor is in an inner circular muscle. Ultrasonic diagnosis by a miniature probe is useful for qualitative diagnosis and to decide whether endoscopic treatment of an SMT is possible or impossible. If necessary, biopsy should be performed. Conventional endoscope observation is also important when we perform the primary diagnosis of SMT. An endoscopic therapeutic procedure for SMT seems to be possible using the method that is followed for endoscopic mucosal resection of esophageal superficial carcinomas. We think it is most important to make a clear diagnosis of SMT before undertaking any treatment.
