前投薬に経口デクスメデトミジン塩酸塩とケタミン塩酸塩筋肉内注射を使用して良好に管理しえた自閉症で多動性障害の青年男性の1症例

A Case of a Young Male Patient with Autism and Hyperactivity Disorder who was Well Managed using Oral Dexmedetomidine and Intramuscular Ketamine as Premedication

要旨

　拒否が強い自閉症で多動の青年男性患者に，ジュースに混ぜた経口デクスメデトミジン塩酸塩（dexmedetomidine：DEX）とケタミン塩酸塩（ketamine：KET）筋肉内注射で鎮静を行い，レミマゾラムベシル酸塩で歯科手術を完遂し，良好に管理できた症例を経験した。DEXを混ぜたジュースは飲みやすく，術前の口喝や不安を和らげ，抵抗やこだわりの強い患者にも有効な方法となる可能性がある。

　We experienced a case that an autistic hyperactive male patient with strong refusal was well sedated with oral dexmedetomidine（DEX）mixed in juice and intramuscular ketamine as premedication and was able to complete dental surgery during general anesthesia with remimazolam. Juice mixed with DEX is easy to drink, relieves mouth dryness and anxiety before surgery. Therefore, oral premedication of DEX mixed with juice has the potential to be an effective method for patients with strong resistance and persistence.
