【目的】わが国における小児構音障害に対する言語聴覚療法のエビデンスを明らかにするため,リサーチ疑問「わが国で行われている言語聴覚療法は,小児構音障害に対してどのようなアウトカムをもたらすか」についてシステマティックレビューを実施した.
【方法】適格基準を設けて文献を検索し,1次・2次スクリーニングを実施した.次いで,採択した文献についてバイアスリスクの評価項目を明確にして品質評価し,結果を統合した.
【結果】1次スクリーニングで169件中71件,2次スクリーニング後に25件の文献が採択された.研究デザインは単一事例研究デザイン19件,ケースシリーズ6件であった.介入方法は系統的構音訓練が多く,介入後はすべての研究で肯定的な結果が示された.
【結論】小児構音障害への介入の有効性が明らかとなったが,効果判断に必要なエビデンスは十分には得られなかった.今後,本領域での質の高い研究の増加が望まれる.
Objective:To clarify the evidence on the effectiveness of speech-language therapy for child speech sound disorders in Japan, a systematic review was conducted to examine the research question, “Is speech-language therapy in Japan effective for child speech sound disorders?”
Methods:Studies were searched according to eligibility criteria, and then filtered through primary and secondary screenings. Selected studies were evaluated for risk of bias, assessed for quality, and the results were synthesized.
Results:Of 169 articles, 71 were selected in the primary screening, and 25 were included after the secondary screening. The study designs included 19 single-case studies and 6 case series. Most interventions involved traditional articulation therapy, with all studies showing positive outcomes.
Conclusion:While the effectiveness of interventions for child speech sound disorders was demonstrated, the evidence was limited by the predominance of single-case studies and case series. There is a need for more high-quality research in this area.
