Abstract Purpose:Paracentral acute middle maculopathy(PAMM) is an ischemic lesion of the middle retinal layers due to impaired perfusion within the retinal deep capillary plexus(DCP). Recent reports have demonstrated an association between PAMM and migraine episodes. We present a case of PAMM in a healthy adult woman with a history of migraine.

Case:A 33-year-old woman with a history of migraine and visual aura presented with sudden photophobia followed by a paracentral scotoma in her left eye. Her best-corrected visual acuity was 1.2 in both eyes. Fundus examination of the left eye revealed retinal hemorrhages, peripapillary subretinal hemorrhage, cotton-wool spots, and retinal whitening extending from the parafoveal region toward the temporal retina. Optical coherence tomography(OCT) revealed perifoveal hyper-reflectivity involving the inner nuclear layer. OCT angiography revealed a flow deficit in the DCP, extending from the foveal avascular zone toward the temporal retina. Automated visual field testing revealed paracentral scotoma corresponding to the area of retinal whitening. At the 2-month follow-up, the fundus changes improved, however the visual field defect persisted.

Conclusion:We report a case of PAMM in an adult woman with experiencing migraines. Retinal microvascular alterations related to migraine may have contributed to the disease onset.
