Abstract Purpose:Gas tamponade in phacovitrectomy for rhegmatogenous retinal detachment(RRD) has been reported to cause anterior shift of the intraocular lens(IOL) and postoperative myopia. SF6 and C3F8 are used for gas tamponade in phacovitrectomy. In this study, we compared the postoperative refractive error and IOL position using the two gases in simultaneous cataract and vitreous surgery in the RRD.

Subjects and Methods:Patients underwent phacovitrectomy for RRD with non-macular detachment between December 2021 and February 2024 at our institution.IOL was an acrylic NX-70S(Santen) 3-piece lens with an optical diameter of 7 mm inserted. The IOL position was evaluated as the difference between the preoperative distance from the posterior surface of the cornea to the center of lens thickness and the postoperative distance from the center of the posterior cornea to the anterior surface of the IOL. The patients were divided into two groups, SF6 and C3F8, and their postoperative refractive error and IOL position were analyzed at 1 month and 3 months postoperatively. The refractive error was defined as the difference between the objective refraction value and the predicted refraction value.

Results:There were 60 patients with 60 eyes in total, 30 patients with 30 eyes in the SF6 group, and 30 patients with 30 eyes in the C3F8 group. The refractive error was −0.45±0.49D in the SF6 group and −0.74±0.52D in the C3F8 group at 1 month postoperatively(p<0.05), and −0.47±0.50D and −0.64±0.46D at 3 months postoperatively, in that order(p=0.20). IOL position was 1.01 mm and 1.07 mm(p=0.22) at 1 month postoperatively and 1.01 mm and 1.03 mm(p=0.72) at 3 months postoperatively in both groups, but there was no significant difference between the two groups.

Conclusion:After phacovitrectomy for RRD, refractive error at 1 month postoperatively was significantly myopic in the C3F8 group, but there was no difference between the two groups at 3 months postoperatively.
