要旨
目的:産後3日目に転院するセミオープンシステムを利用した母親の体験を明らかにする.
方法:A県内のセミオープンシステムを利用した母親13名に半構造化面接法によりデータを収集し,修正版グラウンデッド・セオリー・アプローチを用いて分析を行った.
結果:母親の体験は,【やむを得ない選択】から始まり,連携病院に【慣れない環境での身の置きづらさ】を感じ,連携病院と地元病院をそれぞれ【身の頼り所と心の拠り所】と認識していた.転院時には【不安に勝る転院の喜び】を感じ,転院後は【地元での心身の支え】を認識していた.
結論:産後3日目に地元病院に転院することは,母親にとってセミオープンシステムを利用する長い期間を乗り越える原動力であり,住み慣れた地域に戻ることで,妊娠期から関わりのある助産師から時間をかけてケアを受けることができていた.
Objective: This study aimed to clarify the experience of mothers using a semi-open system, including moving to the place they had pregnancy health check-ups on the third day after childbirth.
Methods: Data were collected from 13 mothers using a semi-open system in A Prefecture by semi-structured interviews and analyzed using modified grounded theory approach(M-GTA).
Results: The experience of mothers began with "an unavoidable choice", and they felt "difficulty being in an unfamiliar environment" at the partner hospital. They recognized the partner and local hospitals as "a place of personal dependence and emotional reliance," respectively. During local hospital transfer, they felt "the joy of the transfer that outweighed anxiety" and after the transfer, they felt "physical and mental support in the local community."
Conclusions: Moving to a local hospital after childbirth helped mothers in overcoming the experience of using the semi-open system. Moreover, by returning to a familiar area, they could take the time to receive care from a midwife who has been involved since the pregnancy period.
