消化管出血の患者では，吐血や下血を主訴に受診する場合が多いが，特に小児患者では，基礎疾患や年齢により消化管出血をきたす原因は多岐にわたる。重症例では頻脈や血圧低下を伴い出血性ショックへと移行する場合もあるため，出血源の適切な評価と早期介入が重要となる。また，小児の消化管出血に対する内視鏡的治療は侵襲度が高くリスクも伴うため，内視鏡の必要性とリスクについて患者・家族に十分な説明を行うとともに，処置中の全身管理や予定している止血処置がうまくいかなかった場合の対応についても考慮しておく必要がある。

　Patients with gastrointestinal bleeding may present with hematemesis, melena, or hematochezia. The causes of gastrointestinal bleeding in children vary by underlying disease condition or age group. In severe cases, bleeding may progress to hemorrhagic shock with hypotension, so appropriate assessment and early intervention for the bleeding lesions are essential. Because endoscopic treatment of gastrointestinal bleeding in children is invasive and risky, patients and families should be well informed about the need for endoscopy and the associated risks. The endoscopist should always consider the next treatment when endoscopic intervention is not successful.
